About the Veterans Plan

The Veterans Plan form collects a veteran’s service address, name, years served, annual income (auto-formatted), and requires uploading the Certificate of Release or Discharge from Active Duty, the official Department of Defense document summarizing military service and discharge status. An alphabetized SPD code input with search auto-fills the narrative reason or prompts for a discharge reason if not found. Veterans also select their military branch, may add an explanation if the discharge is not honorable, and must review a disclosure describing CPFS’s coordination with state and federal lifeline programs.

All Military personnel who submit a Military Benefits application automatically grants approval for our organization and partners to search for available statewide and federal benefit programs across the country that can potentially satisfy this requirement. If the applicant qualifies for a state or federal benefit program, we will work with the program to provide appropriate services. If an applicant doesn't qualify for a state or federal benefit program, we will review the application for our Military Benefits Program. State and Federal benefit program decisions have no impact on receiving approval from our Military Benefits Program.
